Do you have something protecting the bulb from splashes?
If so then 4" is fine. Even at 9", my Wrasse has caused one of my bulbs to explode. Picking glass out of the tank is not easy.

Depends on the bulb. I have my 250w ushio's at 8" while my 400w I run at about 14".
