GOOD TIMING

Gotta go with:
1. No Quarantine 2. Innappropriate choice for tank or tank-mates. I think review of other threads proves both and haven't we all made these mistakes atleast once? |

The question was 'mistakes that cause an ailing fish.' You see, I took the question literally. Quarantine or not doesn't cause an ailing fish. It is an opportunity to cure a diseased, injured or sick fish, but quarantine one way or the other doesn't cause an ailing fish. Your 1) is a part of the environmental category and seems good.

Not observing the fish at the store to make sure it's not ailing when you buy it.
Poor tank maintenance.
