I am very sorry. I hadn't noticed or seen this post. I apologize for the delay in responding.
The condition either is leading to or has led to an infection. Without more evidence you may assume a bacterial infection. The fish will need to be moved to a hospital tank and treated with an antibiotic. My fav is Maracyn Two for Saltwater fish. But Maracyn Two for freshwater fish will be okay.
When the fish heals, it will not improve in the original tank unless conditions there improve.
To find out how the fish got to this stage, it will take determining if the fish was injured, malnourished, or in less than good water quality. If you want help in discovering which of these (or any combo of them) may be the underlying cause, then you'll need to list a lot more information about your system, chemistries, test kit results, etc., etc.

Is this the same small tank that had that large Tang (Paracanthurus hepatus) in it? Is that Tang still there? I think quite some time ago I recommended that the Tang be moved to a larger tank or be given a new home in a larger tank.
Overstocking is a cause that leads to poor water quality and thus to this fish's condition.

I got rid of the blue hippo tang some time ago. This is a new problem.... Going to spend some time on the tank this weekend so I will keep you posted.
