Either Hyposalinity or a Cu treatment. If its MI that is. IDK about anyone else but I can't see the pictures so i'm not sure about hyposalinity or Cu if its not MI.

My apologies for those that I have caused confusion and inconveniences...
I have performed a heavy formalin bath and quarantine w/ prazipro. I am thinking its some form of Gill Flukes, so I thought no harm done if I am already stressing the fish out by QTing.. I'll do formalin...
I cannot see the health of the fish because its hiding under a rock in my QT (Yes I have a rock b/c it can help stabilize this tank and I have this QT tank running 24/7 w/ prazipro)

Thanks for those good photos. There are many ailments of the gill structure. This particular display I have seen before, but never attributed it to any one particular pathogen or water condition.
Check all water quality issues. I may be able to spot the cause if you are willing to supply a lot of additional information:
How old is your tank? When did it originally cycle?
What is the size (dimensions and gallonage) of your aquarium? Does the gallonage include the volume from the sump and any refugium?
Do you use carbon, skimmer, mechanical or other chemical filtration? List all along with their maintenance.
Do you have live rock in the system? How many pounds or amount and where is it put?
Other than live rock hitchhikers, list all marine life & sizes in the tank (fish, inverts, corals, clams, snails, crabs, shrimp, etc.).
Do you use a quarantine tank and procedure?
Foods you use and feeding schedules.
How long have you had this fish? If the fish was recently acquired (6 weeks or less), two more questions: Did you treat it or give it a dip before it went into the aquarium? How did you acclimate it – what procedure?
Do you use any vitamins? Fat additives? Any elemental or other additives? Please list all.
Chemistries – Do you test for Phosphate, alkalinity, calcium and magnesium? Please give actual current numbers for everything. I would like test kit test results, not dip stick results and actual numbers even if you think all is fine/okay. Have these numbers been changing lately?
Water parameters – please give actual numbers (pH and your pH range, salinity or sp. gr. & range, temperature range)
Do you see any of the following in your system: hair algae; micro algae; cyanobacteria growths (red slime algae); dinoflagellate (zooxanthellae) growths; brown algae; diatom growth; slimes; off-colored patches on rock or substrate that are not coralline; etc.?
Water changes (how much and how often).
What is your source water? (Tap water, RO water, DI water, RO/DI, distilled, etc.)
List what you added or taken out of your aquarium system (living, decorations, and equipment) during the past 6 weeks.
Maintenance schedule. What have you done lately?
Keep in mind that even with the above information, I may not be able to help. Although I will say that the photos seem to show fusion of gill lamellae.

Parameters have been stable, and the fish has been in the tank since January. he was the sole survivor from an ich outbreak in january...

phosphate 0.5 (dealing with some algae... but its maintained)
Just recently added phosguard to help lower phosphate, but this problem was prior to adding phosgard
The chromis eats very little flakes, but mainly mysis and brine shrimp
Tank started in early december, cycle was complete mid-late december
its a 29G biocube, w/ the biocube skimmer
all 30lbs of LR in the display
10 snails 4 hermits 1 pistol shrimp 2 urchins
At that time, i dripped to acclimate the fish, now i have a permanent QT for all fishes that come home
Feeding is done around 4-5pm daily, never missed a day
I have done a calcium.. its around 450.. never dosed, using tropic marine pro-reef
I have some hair algae, and thats about it
water changes 10% every other week
No decorations added, just a few QT'ed frags of Zoos, frogspawn, torch, shrooms
