Fish only tank? What is the volume of water?
For fish only, and with experience, I assume you tested ammonia, nitrite, nitrate, and pH, and they're all perfect (all 0 except pH is 8.2 and nitrate might be <20ppm)?
So I'm new here, but not new to saltwater fish. I have a small humahuma about 2 inches long who has had both of his eyes cloud over at the same time, but it isn't hazy its more like a blue (like the kind associated with cataracts I would say)
I feed him a various mix of food including krill, squid, mysis shrimp, brine, and also flakes, thinking it could potentially be a nutriton problem I started offering ghost shrimp and all the frozen food is soaked in vitamins before feeding.
He is in a tank with a Auriga Butterfly, a False Percula Clown and a Pixy Hawkfish, all of which are healthy and have no problems. None of which are picking on him, and water levels are good. I did read today on the forum about testing the calcium, alkalnity, and magnesium which I will be doing tonight.
He also doesn't seem to be growing. I've had him for about 2 years and this wasn't a problem until about 9 months ago. He is loosing weight and I have been hand feeding him, worrying about him starving. He has an appite just cant see the food, he does well with the ghost shrimp though cause he can see the movement.
I have not added any new fish and he is was the most recent addition... 2 years ago![]()

That fish usually grows pretty quickly from juvenile to sexual maturity, so there is definitely something wrong. I think you're likely right about the nutrition. It is either wrong foods, not enough food, or something missing from the diet.
For now, the best would be to isolate the fish in its own hospital tank. Feed the fish with foods soaked in vitamins as you have, but also add Vitamin D and A. That is, cod liver oil, of the purest you can find from your nearby health food store. Both these vitamins support eye growth and repair.
Also I want you to de-worm the fish. Just in case foods are okay, there may be an intestinal infection or worm. The food should also contain Praziquantel. Follow the directions given here: Strange Excrement from Fish -or- How to be a Super Pooper Snooper
Also, while feeding the fish the above foods and meds, I want you to give the fish a treatment of a certain antibiotic. I want to be sure this is not a bacterial infection. Don't worry about why the other fishes don't display this -- when it comes to this sort of thing, not all fishes do display infections -- it is a matter of the genetic make up of the fish and water conditions.
Don't be hesitant in assuming the water isn't as good as it can be. Make sure in the quarantine tank that the best water is there. If you don't have a running biological filter to put on the quarantine tank, then be prepared to change the water two or three times a day.
Anyway, treat with Maracyn Two for saltwater fishes. If you can't find this, then use Maracyn Two for freshwater fishes. Either is okay. When you use this medication, I want you to double the recommended dosage that you find on the medicine. This antibiotic is special since it goes inside the fish, through the skin. PLEASE don't try to substitute this with another antibiotic.
Try to take a photo if you can, and post it here. Keep us informed about the progress. Try to move as quickly as you can with the above.

I've had good luck w/ kanamycin via feeding for bacterial related eye hazing.

My recommendation of Maracyn Two for Saltwater fishes was not just for eye bacterial infection, but also to address a possible intestinal infection.
