Exclamation Need help to ID sick puffer

I have a small Stars & Stripes Puffer. All water parameters are optimal. However it has white spots but its not ick. When I shut off the filters etc. you can see the little critters swimming around at the top of the water. The puffer seemed fine up until two days ago. Now it has excess slime and looks stressed. The upper lip looks pulled back exposing the upper teeth. Any ideas what this might be?
