Your clean up crew will usually do that for you. There are some cases in which you can siphon some sand out but that's usually not the case. If you get a lot of algae growth or something that doesn't look right you can just siphon it out.
I only had to siphon some sand out once in 4 months.
29 Gallon(Biocube),1 Clownfish, 1 Cherub Angelfish
1 Skunk cleaner shrimp,1 Green mushroom
Not dumb at all, my roots are/were freshwater too so I completely sympathize with your dilemma.
Here's the bottom line, or so I've found so far... You should never have to siphon clean your tank again. Either you have sufficient filtration, cleaning crew and water movement or you don't.
If you have lots of "stuff" accumulating on the bottom then you probably don't have enough of one, or more, the afore-mentioned.
